1. Information we collect
When you use our website or complete an assessment, we may collect your name, email, phone number, date of birth, state of residence, responses to clinical screeners, and any details you choose to share with us. If you become a patient, we also collect health information needed to provide care (medical history, medications, treatment notes, and billing details).
2. How we use your information
We use your information to deliver clinical care, schedule appointments, communicate with you, process payments, comply with legal obligations, and improve our services. We do not sell your personal information or PHI.
3. How we share information
We share information only with people and vendors who help us deliver care — clinicians, our electronic health record, secure communications, payment processors, and analytics vendors that have signed business-associate agreements where required by HIPAA. We may also disclose information when required by law (e.g., subpoenas, mandatory reporting, or imminent safety concerns).
4. Security
Assessment responses and clinical records are stored in encrypted systems with access limited to authorized personnel. We use TLS for data in transit and AES-style encryption at rest. No system is perfectly secure; we work continuously to reduce risk.
5. Your rights
You may request a copy of the information we hold about you, ask us to correct it, or request deletion subject to legal record-retention requirements. Patients also have specific rights under HIPAA — see our HIPAA Notice for details.
6. Cookies & analytics
We use a minimal set of cookies and privacy-respecting analytics to understand how visitors use the site. We do not use third-party advertising trackers on pages that collect health information.
7. Children
MindCare provides care to adults only. We do not knowingly collect information from minors.
